As soon as you, the customer, has signed the quote, it will need to be emailed directly to one of the Undock representatives. To ensure a smooth transition from your previous carrier, the following documents are needed:
LOA form: for all local and hosted fax numbers
RESPORG form: for toll-free numbers
Current Phone bill from the current/losing carrier
* LOA (Letter of Agency) is a signed agreement that Undock has permission to submit the local or fax phone numbers from the clients current telephone provider to Undocks porting center.
*RESPORG form is a signed agreement that Undock has permission to submit any toll-free numbers from the clients current telephone provider to Undocks porting center.
*LOA forms need to match their current provider's billing information from the company name, account information and name on bill. The LOA form will be filled out in advance for you by your dedicated Undock representative. All you need to do now is review the form, sign and date the agreement.
*If there is more than one number you are porting, make sure all those phone numbers are with the correct losing carrier. For instance, if you are porting four numbers, one of those numbers may be with a different carrier than the other three phone numbers. Therefore, we will need the phone bill for that number that's with another carrier.
